The patient came from the Pacific Islands and walked bare footed. These lesions are thought to be due to sensory loss combined with mechanical trauma (chronic irritation)
Note: Lepromatous leprosy is present when immunity is reduced. By contrast with the tuberculoid forms of leprosy where the lesions are granulomatous and circumscribed – understanding the Ridley-Jopling classification and the clinical appearance of lesions is useful in making sense of lesions and has a teaching value for all those learning dermatology. Like Syphilis in many countries, Leprosy can also be confused with othe conditions (a kind of “Great Pretender”) and is integrated in the Specialist degree of Dermatologists (Dermatologist and Leprologist for example in India: Indian Association of Dermatologists and Leprologists (IADVL))
